IRS FORM W-9 INSTRUCTIONS

This form is required in order to verify an owner's name, address, tax number, and tax classification.

2. Open the attachment.
3. Save the form to your computer.
4. Open the saved form on your computer.
5. Type in your information.
6. Print the form, sign, and date it.
7. Scan and save the completed and signed form to your computer.
