DIRECT DEPOSIT

For your convenience an optional service offered by GulfMark Energy is Direct Deposit of royalty payments through EFT-Electronic Funds Transfer. This service is available once you have received your first royalty payment.

Direct Deposit advantages:

• Receive funds quicker; no mail service delays
• Eliminates problems associated with lost or even stolen checks
• Reduces paperwork and trips to the bank

Direct Deposit requirements:

• Only available for accounts drawn within the United States
• If a voided check is not available you may obtain a letter from your banking facility providing the appropriate payment instructions
• Trusts: May only be paid to the account in the name of the trust. However if funds are being deposited to the Trustee's personal account upon receipt of documentation from the bank confirming they have trust documents on file, an exception may be made.

Direct Deposit enrollment instructions:

1. Click on this link to download the Direct Deposit enrollment form.
2. Open the attachment.
3. Print the form.
4. Fill out the form following the instructions on the form.
5. Include a voided check or savings deposit slip from the account where the funds are to be deposited.
6. Please mail, e-mail, or fax the signed direct deposit enrollment form along with the voided check or savings deposit slip to the mailing address, e-mail address, or fax number listed below.
