AFFIDAVIT OF HEIRSHIP FORM INSTRUCTIONS

This form provides information about a deceased person, including family and marital history.

1. Click on this link to download the Affidavit of Heirship form.
2. Open the attachment.
3. Print the form.
4. The form must be completed by an ATTORNEY OR A DISINTERESTED PARTY (Someone not related to the deceased). Please note: any person listed on the Affidavit as an heir of the decedent must not fill out the Affidavit; otherwise, it will be returned for re-completion and recordation.
5. Once completed, form must be executed in the presence of Notary Public.
6. Please have the Affidavit recorded in each county/parish where the well(s) are located. Since recording the document can be done by mail you may want to contact the county courthouse for further instructions. Please note: GulfMark Energy does not pay the recording fee.
7. When all steps are complete, please mail, e-mail or fax a photo-copy of the RECORDED Affidavit of Heirship and Death Certificate. Also, please include contact information (address and phone number) as well as the current addresses for all heirs.

Important Note:GulfMark Energy requires that you furnish photo-copies of recorded documents; we cannot accept copies of documents that are not recorded. IT IS NOT NECESSARY TO RECORD DEATH CERTIFICATE.

This form is not intended as a substitute for the probate of a decedent's estate. GulfMark Energy, Inc. reserves the right to request all documents in addition to or in support of this form necessary to verify ownership.
